All Employee Benefit elections must be elected prior to your effective date of coverage.

By utilizing In-Network (or contracted) physicians and/or facilities (Allied), you will be able to keep your out-of-pocket costs down and receive the highest benefits available. To verify whether a physician/facility is contracted, you can either go to the website www.alliedbenefits.com/Members, or call customer service at 855-424-3032. All information is on your ID card.
