Member ID Card

Your Medical ID Card is expected to be mailed to you on December 27th 2025. Until you receive your Member ID Card, you can have a temporary ID Card via the options of My Allied benefits and My Allied Benefits Mobile App to have your Temporary ID Card on January 1, 2026.

Member ID Card Examples

As explained in the prior page on the Medical ID Cards, your Medical ID Card contains key information about you and your coverage for the physicians and facilities when verifying benefits and filing for the services provided. Anytime you visit your doctor, hospital, or other health care provider, remember to show them your Medical ID Card or your Digital ID Card.  Please remember, your Medical ID Number is listed on the front of your ID Card, and you should include the “alpha” listed with the numbers.  Over the past year, we have discovered some physicians and facilities use the wrong Group Number when filing claims. The illustrations below identify which group number to use with which provider. The illustrations below provide a description for the service providers to identify and use their respective group number. Group Number NGC001 is for Blue Cross Blue Shield Service Provider in the State of Arizona.  If you use the BCBS of AZ PPO Statewide network to find a physician, hospital, urgent care, lab facility, etc., these participating providers are to use group number NGC001. (Please refer to the Red Box on the illustrations below).Allied Group Number A25226 is for providers and facilities outside of the BCBS of AZ network and all Out of State providers and facilities. (Please refer to the Purple Box on the illustrations below).
